The Episcopal Church Annual has listings by diocese under the "Health & Welfare Agencies" section.  Your local church may have a copy of this directory in its office, or you may wish to order a copy from Episcopal Books and Resources (EBaR).  They can be reached at 800-903-5544 or http://www.episcopalbookstore.org/

Individual dioceses may also have information available on communities in their areas.  Here is the link to The Red Book online, where you may look up diocesan and church contact information:  http://www.theredbook.org/
