Initial Registration Form This form can be filled out online and transmitted to CDO electronically. Or, download the form, complete it and mail to CDO. CDO will email a User Name and password so that you can complete the registration online. Long Registration Form This form is for registration of Episcopal clergy, seminarians and lay professionals. It contains general instructions. It should be used in conjunction with the Ministry Skill/Experience Manual. Ministry Skills and Experience Manual This manual contains a hierarchy of skills/experiences from which a registrant or a parish/institution can select relevant skills to add to either the Personal Profile or the Parish/Institutional Position Profile. Lay Professional Certification Form This form is required for registration of lay professionals working in the Episcopal Church. Anglican Bishop’s Consent for CDO Registration This form is required for registration of clergy members of other churches/provinces of the Anglican Communion and must be signed by that cleric’s bishop of record. Episcopal Bishop's Consent for Registration of Anglican Clergy Abroad This form is required for registration of clergy members of other churches/provinces of the Anglican Communion and must be signed by an Episcopal bishop who knows of the clergy person’s ministry. Episcopal Bishop's Authorization for Registration of ELCA Clergy This form is required for registration of rostered Lutheran (ELCA) clergy and must be signed by an Episcopal bishop. ELCA Clergy Registration Packet These forms are required for registration of rostered Lutheran (ELCA) clergy with the CDO.
